How to qualify:
All proposed financial assistance grants are reviewed and must be approved by the two Co-Chairs of the Social Services Committee and subsequently by the Philoptochos officers who sign all checks: National President and Treasurer, or designee.

What if I’m not an Orthodox Christian?
Non-Orthodox persons who seek our help are provided with helpful next steps and referred to local resources.

Requirements of applicants:

1. Application for Assistance Form

    Complete and sign the fillable application, linked above. You may fill out the digital copy or print it out, then scan. Send the form to PauletteG@philoptochos.org once finished!

2. Consent for Release of Information Form

• • Consent Release Form
    This gives us permission to contact secondary sources (ie. medical providers, etc. landlord, utility company, etc.) in case we need to confirm details, and if needed, to request more pertinent information

3. Interview to evaluate whether and how we can help 

4. Additional Documentation

• Recent photo of yourself, and if relevant, photo with your family
• Household income (ie. paystubs and/or copy of tax return, unemployment benefits, other support)
• Household expenses
• Any other help you are receiving, have received, or requested
